Metrogyl® Denta (Metronidazole Gel)
METROGYL Denta is an off-white containing Metronidazole Benzoate BP equivalent to 10mg of metronidazole per gram. Chlorhexidine Gluconate Solution BP (20%) 0.25% w/w as preservative. The dental gel is designed for application to periodontal area.
Usage
Metrogyl denta is to be used in the treatment of chronic periodontal disease as an adjunct to conventional therapy.
Metronidazole is an antibacterial active against those organisms that are predominant in the subgingival flora in adult periodontitis. Metronidazole has a bactericidal effect against Bacteriodes spp., Fusobacterium, Wolinella, Spirochetes and other obligate anaerobic organisms, but does not affect aerobic bacteria. Also some facultative anaerobic bacteria, such as Actinobacillus actinomycetes are sensitive to the concentrations of metronidazole obtained after local application of Metrogyl Denta. Development of resistance against metronidazole is seldom reported, and only with high doses after long-term use. Metronidazole does not affect the commensal flora.
Dosage and administration
Metrogyl Denta is applied to the periodontal area twice daily. Dosage is individual, dependent upon the number of teeth to be treated. The average course of treatment is 2-4 weeks.
After thorough tooth brushing wash your mouth with soda solution and wipe your gums with a dry cotton ball. Squeeze out a small amount of gel onto your toothbrush and apply gel with toothbrush to your gums and interdental spaces. Next thirty minutes after the application of the gel you should not wash your mouth or eat anything.
Indications
It is used for the treatment and prevention of infectious disease of oral cavity namely:
- Acute gingivitis
- Acute necrotizing ulcerative (vicent’s) gingivitis
- Chronic gingivitis and gingival inflammation
- Chronic hydropic gingivitis
- Chronic hyperplastic gingivitis
- Chronic atrophic (desquamative) gingivitis
- Chronic periodontitis
- Periodontal abscess
- Recurring apthous (ulcerative) stomatits
- Juvenile periodontits
- Toothache of infectious origin.
Contraindications
Known hypersensitivity to metronidazole and chlorhexidine.
Precautions and warnings
In some patient metronidazole may have an effect similar to that of disulfiram on the metabolism of alcohol; resulting intolerance symptoms.
Side effects
Because of the low plasma concentrations after local application of the dental gel, the risk of systemic side effects is low. The most frequent side effects are local and occur directly in connection with the application, such as bitter taste and temporary local tenderness. Headache has been reported.
Adverse reactions
After local application of Metrogyl Denta the risk of systemic side effects development is insignificant. Metallic taste in mouth, headache, allergic reactions (pruritus, rash) may, however be observed.
Drug interactions
In local application of Metrogyl Denta the interaction with other drugs is minimal, but the possibility of interaction cannot be absolutely eliminated.
Therefore one should be careful when prescribing Metrogyl Denta along with following drugs
- Warfarin and other coumarin anticoagulants-Metronidazole enforces anticoagulant action which results in the increase of prothrombin time.
- Disulfiram: Simultaneous use of Metronidazole increases toxicityof this preparation which may result in development of neurological symptoms.
- Antibacterial activity of metronidazole decreases due to accelerated metabolism of metronidazole when used simultaneously with phenytoin and phenobarbitone.
- Cimetidine inhibits metabolism of metronidazole which may result in higher concentration of metronidazole in serum.
Use in pregnancy
Metronidazole should not be given during the first trimester of pregnancy unless it is considered essential.
Lactation
Metronidazole is excreted in milk but the risk of affecting the child seems unlikely with therapeutic doses.
